All players and cheerleaders must submit the required documents listed in the table below to the PVYFC Secretary via our secure DropBox link before the first day of practice. The physical must be documented on the official PVYFC physical form. If all documents are not submitted by August 3rd (heat acclimation day), your child will not be permitted to practice.
Please note: new participants must submit all listed documents. Returning families are not required to provide a report card or birth certificate.

The PVYFC Athlete Code of Conduct will be reviewed with each athlete by the coaching staff. Please take time to review it with your child as well.
