It is Mandatory to attend one of the following dates so that your child can be fitted for uniforms and football equipment.  Cheer will also be available for uniform fittings during this time.

Please bring the following:

BFCA Liability Waiver

BFCA Medical Waiver 

BFCA Parent Consent Form (3pgs)

Birth Certificate if it is child's first year participating in BFCA

$200 football equipment deposit check (will be returned/destroyed upon return of equipment)

$100 uniform deposit check for Cheer (will be returned/destroyed upon return of equipment)

$150 volunteer deposit check (will be returned if parent/guardian fufills 3 hrs of required volunteer hours)

$100 Fundraising fee (You will receive your (5) Charity Mania tickets -$20/ticket for 17-week NFL football season)
