Below are some key dates and information.  It is very important to start planning summer vacations around some key dates, schedule your child’s sports physical now and plan to keep a copy of your child’s last report card from this school year.  

Plan to make 2 copies of your child’s final report card and set it aside.  District offices don’t open until mid-August so getting copies will be problematic during the summer.

Sport’s physicals need to be dated after 1/1/2019 and completed on the 2019 Physical Fitness and Medical History Form.  

Each child requires the following paperwork to be completed and turned into ONE of our MANDATORY Registration Events.

Please bring the following documents to one of these events  NO EXCEPTIONS (2019 Forms will be uploaded to the site as soon as they are available):

*Signed 2019 Player/Participant Contract-no strikeouts/mistakes and signed by both the parent AND the player.

*Signed 2019 Physical Fitness & Medical History Form - page 1 completed and signed by parent, page 2 completed and signed by a licensed medical professional and must be dated after 1/1/19.  Again, no strikeouts/mistakes are allowed.

*Current 2x2 photo of your child’s face (no obstructions to their face - glasses, hats, etc). Must be on photo paper - not printer paper.

*2 copies of the final full year report card for the 2019 school year (ok to turn report cards in after they are available).

*Signed and completed Volunteer Commitment Form (with $100 volunteer deposit) and signed Parent Code of Conduct Form.

*New players must also bring: 1 copy of your child’s birth certificate.

Please mark your calendars for the following key dates:

*August 1st - First day of practice & Equipment Handout, Carlmont High School - Exact time TBA

*Month of August plan on practices being Monday through Friday - Carlmont High School - Exact time TBA

*Saturday, August 24th - Mandatory Weight Certification, Santa Clara Fairgrounds - Time approx. 9:00am-1:00pm  TO BE CONFIRMED  

*Sunday, August 25th - First game - Location and Time  TO BE CONFIRMED
