Codes of Conduct
The following forms outline MOT Little League's expected codes of conduct. All parents must understand and practice the principles of behavior outlined in the Parent Code of Conduct document. Additionally, managers and coaches are expected to exemplify true sportsmanship while coaching. The Coach's Code of Conduct outlines those principles.

Volunteer applications
The league is always looking for volunteers. Please consider volunteering your time to the league today. All coaches, managers, umpires and any parent volunteers must submit a Volunteer Application Form along with a copy of their Driver's License and be approved by the Board before taking the field. In order to submit your application, email our Volunteer Coordinator at [email protected] with your name and role.

Medical Release forms
Please fill out and return the Little League medical release form to your team manager before practice begins. These forms are used in case the parent is absent from a game or practice and an accident occurs requiring medical attention.
