To be certified for each season, Players are required to provide the following:
1. A photocopy (not photograph) of their birth certificate
2. A current SPORTS physical. This physical must be dated to carry your player through the end of the season (end of November). If you need a physical form, you can use the CTYFL Physical Form
3. A signed CTYFL Spectator Behavior Form
4. A signed CTYFL Waiver of Liability, Medical Release, and Indemnification Agreement
In the event a player is removed from play for a suspected head injury, the Return to Play form is required
