Please upload a current picture of your child and a copy of their birth certificate directly to your online account-INSTRUCTIONS HERE

With the same instructions, scroll down to the bottom and you will see a section where you can upload the filled-out documents to your account that you filled out above (Club Agreement / Player Medical Release/ Parent Code of Conduct / Player Code of Conduct).  After you upload each document you will need to scroll down click the "UPDATE" button at the bottom right of the page.  Once this is completed your player will be verified.

It is important that any returning players upload the new  documents for the UPCOMING season.  Any documents from prior seasons can be removed by clicking on the trash can icon.  The Medical waiver does not need to be changed / updated unless your insurance information has changed.

2024/2025 Uniform Ordering Information




New Players will be assigned a uniform number so as to not conflict with current players or players in the same age group to allow for potential player movement between teams.
