Football - $275 per participant
Cheer - $255 per participant
Family discount of $20 off the registration fee for each additional participant

You are responsible for purchasing the following required gear:
1.       Black helmet / Yellow/Grey/Black face mask 
2.       Shoulder pads
3.       Practice jersey & pants
4.       Football cleats
5.       Mouth guard with strap
6.       Water bottle (approx. 64 oz.)
NOTE: Please look online for helmets and pads. You do not have to spend a lot to get a quality used product.
 
Pop Warner will provide:
· Home & Away Jersey
· Game pants
· Game socks
· Helmet decals

Required Registration Forms to be provided
 
1.       Copy of Birth Certificate
2.       Report Card of most recent academic year
3.       Physical
4.       Player Contract

Please print and fill out the following forms. They should be turned in prior to the first day of practice (March 1).

1.  2022 PARTICIPANT CONTRACT AND PARENTAL CONSENT FORM  - Make sure it's filled out completely with all vitals, height and weight.
2.  2022 PHYSICAL FITNESS & MEDICAL HISTORY FORM
 - Both player and parent/guardians need to read and sign.

You will also need to provide (by August 1):
1. Birth Certificate - A copy will be fine.  No originals this year.
2. Report card - End of last school year with all 4 quarters (copy)

If you are an adult and volunteering in any capacity (Board member, Coach, Team Parent, etc.) for the 2019 season you will be required to fill out the following form. Please print, fill out (include a copy of your Drivers License and have available to turn in to the President or Vice President.
