It is required by National Pop Warner to collect the following forms at the time of registration;
- Physical Fitness Medical History Form 
- Player Contract and Parent Consent Form 

These forms must be downloaded, filled out and signed, then uploaded into the system during the registration process, you will not be able to complete and pay for registration until the forms are uploaded.
