Injury Tracking Form - Coaches, please have this form available to fill out in the case of any injury (additional forms are needed if concussion is suspected)

Cautionary Statement - This form must be signed by all parents who have a child registered to play ball with NBLL.


Concussion Awareness

One of the risks involved in any recreational sport is the risk of a concussion. North Burlington Little League takes the safety of our children very seriously. This document: Signs & Symptoms of a Concussion provided by the National Federation of State High School Associations gives a brief overview of risks and signs of a concussion.

Our Coaches & umpires are required to register at NFHSLearn.com  to take the free course on Concussion Awareness.

  • Click on "Order Course" at the top of the NFHS site.

  • Set-up your registration (it's free)

  • Read through the tutorial, then start the course.

  • Please make sure that you have set aside about 30min to take the course. Make sure your computer has sound.

  • Upon completion a certificate is awarded. Please email a copy of your certification to NBLL.

In addition to taking this brief yet important course, the following documents are available to record and document a concussion incident: 

  • There are 5 steps to follow when introducing a player back to a sport after a concussion. These steps must be followed, following a doctors written release.
The CDC website is also an outstanding resource for concussion information.
