As part of registration, FLLAA will need to have several documents on file for each player, including:

Tee-Ball, Baseball & Softball:

• Residency and School Attendance Eligibility Requirements
  
• School Enrollment Form
• Medical Release Form
• Code of Conduct (online agreements as part of registration)
