All forms can be downloaded here or within the registration process.
Note: Your child cannot be placed onto a team without a completed Medical Information form on file with Sunrise LL.
Part 3 - Submit residency and birth certificate (may not apply to all players)
Residency: Your player must live within Delta Sunrise LL boundaries or attend a school that's within the boundaries in order to play for Sunrise LL.
Establish player's residency using 3 residency documents OR a completed school enrollment form (this is preferred and also available within the Registration process).
New players who attend a school within Delta Sunrise LL boundaries should complete the school enrollment form if possible. This season, schools are limited to:
- Manlio Silva Elementary
- Christa McAuliffe Middle School
- Podesta Ranch Elementary
- Oakwood Elementary
- Elk Horn Elementary
- Jon Muir Elementary
- Julia Morgan Elementary

Returning players who used a school enrollment form last season do not need to complete this part!
New and returning players who do not attend a school with Delta Sunrise LL boundaries must submit 3 residency documents.
If your player does not attend a school in Delta Sunrise LL boundaries, gather 3 residency documents based on the following table. This applies to new and returning players who are not using the school form.
Quick 1-2-3: #1 - Parent or guardian driver's license showing player's address + #2 - Parent/guardian's Stockton/CalWater water bill showing player's address + #3 - Parent/guardian's PG&E bill showing player's address. Done!
