Please find the forms required for play with JDYFL. All forms should be turned into Coach or the Team Manager before practices or play.
1. ALL athletes need a current physical to participate!! You will need to bring a copy of the physical or JDYFL medical waiver with you to the first practice!! You CANNOT condition or practice without a valid, current physical so be sure to get those done ASAP!!
JDYFL Medical Waiver is found through this link. (**We recommend anyone in the 8th grade get a VHSL sports physical so that they can participate in WAHS spring football workouts. All local doctor offices have them available).
2. ALL athletes are also required to submit their 2022-2023 end of year report card (to verify school information). Alternatively, for Albemarle County Public Schools you can submit a printed page for the "school information" tab in the parent portal. It needs to state the child's name, school, and grade. This will be due in mid-August. BUT the parent portal will close soon to get new year prepped. You may want to print them now before that happens, or contact the school office to get a copy!
3. ALL athletes & parents/guardians are required to sign the JDYFL Code of Conduct found through this link.
4. ALL, we will need to get a copy of your child's birth certificate if they are new this year.
